Insurance
We participate in a large number of medical plans and frequently enroll in new ones. Before you make an appointment at OFC, please contact your insurance company and confirm Dr Mark A. Woods, with the current address, as participating in the plan in which you are enrolled.
 
We will bill your insurance company directly. You are responsible to pay deductibles as well as charges incurred for services that are not covered benefits of your insurance plan. If authorization is required for the visit, but cannot be obtained, you will be responsible for payment. Any outstanding balance owed after insurance payment is made is due as soon as Ozark Family Clinic notifies you of the amount. 
 
Please familiarize yourself with the rules of your plan. Sometimes routine services, such as physical examinations, are not covered. Frequently, prior authorization is required for emergency treatment and referrals. Co-payment amounts may vary. These rules can even vary within a plan each year. It is your responsibility to know and understand the requirements of your particular plan, but we are happy to help address any questions you might have.
